The Flinders Chronic Condition Management Program is a generic set of tools and processes enabling health professionals to partner with their clients to support more effective self-management of their chronic condition, based on cognitive behaviour therapy, problem solving and motivational interviewing techniques. The program is based upon extensive research identifying the importance of self-management and self-management support in chronic condition management and is internationally recognised as being amongst the top 15 models of care for people with high needs and high costs to the health system.
Health professionals have the opportunity to learn about this self-management partnership based on cognitive behaviour therapy, problem solving and motivational interviewing techniques. This involves working with clients and using the Program tools to assess self management behaviours, barriers, psychosocial issues and preferences, to produce an individualised Care Plan that includes problem and goal statements, interventions, steps and responsibilities that align with the client’s values, priorities and beliefs.
